FACILITY WAIVER

I understand and acknowledge that participation in soccer and other athletic activities at HR SportspleX involves inherent risks of injury, including but not limited to sprains, strains, fractures, concussions, and other physical injuries. I voluntarily assume all risks associated with participation, whether known or unknown, including risks arising from the negligence of HR SportspleX, its staff, coaches, volunteers, and other participants.

In consideration of being permitted to participate in activities at HR SportspleX, I hereby release, waive, discharge, and covenant not to sue HR SportspleX, its owners, officers, directors, employees, coaches, volunteers, agents, and representatives (collectively "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my minor child while participating in any activity at HR SportspleX or while on the premises.

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including court costs and attorney fees, that they may incur due to my participation or my minor child's participation in activities at HR SportspleX, whether caused by the negligence of the Released Parties or otherwise.

I authorize HR SportspleX staff to seek emergency medical treatment for me or my minor child in the event that I cannot be reached. I understand that I am responsible for all medical expenses incurred. I represent that I (or my minor child) am physically fit and have no medical conditions that would prevent safe participation in athletic activities, unless disclosed in writing to HR SportspleX staff prior to participation.

I grant HR SportspleX permission to photograph and/or video record me or my minor child during participation in programs and events. I authorize HR SportspleX to use these images and recordings for promotional, educational, and marketing purposes, including on social media, the website, and printed materials, without compensation.

I agree to abide by all HR SportspleX rules, policies, and codes of conduct. I understand that unsportsmanlike behavior, harassment, or violation of facility rules may result in removal from programs without refund. I agree to treat all participants, coaches, staff, and officials with respect.

This agreement shall be governed by the laws of the Commonwealth of Virginia. If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect. This waiver constitutes the entire agreement between the parties with respect to its subject matter.
